Involving patients and the public in the delivery of pharmacy education
Gordon Becket and colleagues explain how pharmacy students at the University of Central Lancashire benefited from speaking directly to patients about their medical conditions
The importance of patient safety and well-being is a vital part of health professional education. With the many reported examples of unprofessional behaviour and poor care being highlighted in the media and through the Francis Report (2013), patient-centred care and the role of the healthcare professional calling has been brought back into focus.
In the Department of Health mandate to Health Education England, “Delivering high quality, effective, compassionate care: developing the right people with the right skills and the right values”1, there is an emphasis on patient and public involvement (PPI). The report says: “To ensure that excellent education is relevant to the needs of patients and the public, the workforce planning system needs to be based on the way people expect services to be delivered.”
It adds: “Education and training of staff should be [given] with the objective of improving the … care delivered to patients and communities. An increased focus should be given to patient and public involvement”.1
This has raised a challenge for both medical and pharmacy educators in providing appropriate undergraduate training, instilling professionalism and values and assessing knowledge, attitudes and conduct adequately.
These needs have been recognised by the General Pharmaceutical Council in its Standards for the Initial Education and Training of Pharmacists,2 which require that all UK pharmacy schools involve pharmacy service users (this group includes patients, the public, carers, and consumers) and this is now becoming an integral part of the new MPharm curricula in UK pharmacy schools.
PPI can cover a wide spectrum of activities from the use of simulated patients to assistance in curriculum and module development. There is nothing particularly new in this since PPI has been used in the training of medical students and some other healthcare professionals since the 1960s. However, traditionally patients have held a more passive role, where they were only used to illustrate unusual medical conditions.3
The new GPhC Standards (especially Standard 5.6) outline the importance of PPI in pharmacy education and emphasise the importance of implementing practical experience of working with patients, carers and other healthcare professionals alongside the theoretical and scientific aspects of pharmacy education.
The DH has also been keen to promote the involvement of patients and the public in decisions regarding the planning, design, development and delivery of local services in the hope that it will lead to an improvement in public health services for patients.4,5
The idea of involving patients in the development and teaching of healthcare professionals has been somewhat controversial, and while there is a considerable literature on the benefits and limitations of PPI derived from medical, nursing and social work training, there is little evidence to demonstrate clear improvement on professional conduct.6,7
The literature indicates that most healthcare professionals in training are in favour of more direct patient involvement with their teaching.8 In an evaluation of patients’ involvement in classroom teaching published in Nurse Education Practice, John Costello and Maria Horne3 claim the success of patient involvement depends on three main factors: the skill of the teacher; the co-operation of the patient; and the willingness of the student to engage in discussion with the patient in a classroom context.
Various formats have been used, including patients giving presentations and facilitating seminars. However, several articles have described potential difficulties with the process of patient involvement, including concerns about patients’ emotional wellbeing and physical stamina.9 There is also the consideration of whether the patients should receive training, and if so, what format it should take.10
Generally, students believe that the patient’s view is necessary and complementary to understanding the theory behind their medical conditions.11 Additionally, patients become “empowered” when they are able to impart their knowledge to a trainee healthcare professional and feel less isolated as a result.6 An improvement in the healthcare professionals’ communication skills has been observed.12
This is of particular relevance to the education of pharmacists, supporting them to develop their skills of talking to people appropriately and with confidence, listening effectively, empathising with patients and supporting patients in their use of medicines.
The Comensus initiative
The School of Health at the University of Central Lancashire (UCLan) developed the Comensus initiative a decade ago to link service users, carers and the wider community with academics, providing students with the opportunity to understand important issues relating to patient care, autonomy, choice and safety. Volunteer members of the public with experience of a wide spectrum of social and health care issues are given training and are involved in activities ranging from small group workshops through to module and course boards and curriculum design.13,14
Over the past year, the pharmacy practice team in the School of Pharmacy and Biomedical Sciences has been working with the Comensus team on delivering PPI workshops with their MPharm students. The School has recently conducted two half day PPI events. During these PPI events the students had the opportunity to attend one of seven workshops. The titles of the workshops were: mental health; breathing difficulties; cancer; diabetes; transgender; drug misuse; and carers.
The workshops were designed to include a question and answer session followed by small group activities facilitated by the service users.
The format for the undergraduate teaching of PPI was developed through initial meetings between the academic staff and the Comensus team. The academic staff then arranged tutorials that brought students from across the four years together to design questions for the workshops. The service users were provided with these questions in the week preceding the workshops, and had time to decide if these were appropriate and to consider their responses.
In general the questions raised were extremely pertinent and there were no issues that the service users and carers were not prepared to discuss. It was considered that having students from all years participating in a group may have brought additional benefits, since there are studies that suggest that empathy may decrease during healthcare professional training.15
The main teaching event took place over a two-week period in March 2013, with over 250 volunteer MPharm students, 20 patients and carers, and eight lecturers and teacher practitioners. The event was duplicated for a second week to cater for the high number of students who wanted to attend. Following the event, students were offered the opportunity to attend a support session with an academic member of staff in case any of the issues had affected them emotionally. Students could also use a discussion board to to ask the service users any follow-up questions.
The school evaluated the PPI event by asking all students, staff and service users to complete a back form. The results of this were positive and highlighted the benefits of including PPI for all participants, particularly by widening student and staff understanding of health needs and medication compliance issues and by changing how pharmacy is seen by consumers. In the evaluation, students were positive about the patients’ contribution to their learning process, and many commented on the unique nature of this learning:
“It was useful for pharmacy students to be able to converse with patients with sensitive issues, which wouldn’t always be possible on placements or when practising as a pharmacist.”

After reviewing the extensive back, the school has decided to integrate PPI across a number of modules. Fourth year students have also participated in a full day event PPI congress with social work students. This involved various third sector healthcare support groups and carer groups and the back suggests that this has further broadened the students’ knowledge and experience.
From the existing literature on this subject, it appears that there is room for growth in “service user” involvement in the education of pharmacy students. Practicalities such as funding and developing and maintaining a positive rapport with patients should be taken into consideration when carrying out further research.
However, in order to develop PPI as an integral part of pharmacy education in the future there need to be more accurate ways of measuring the positive or negative outcomes of such teaching on future attitudes and behaviours in pharmacists. Additionally, the use of PPI in curriculum development will need to be assessed to ensure that this has positive benefits in the educational process.
